emailing forms (with attachments)

What is the best way to send/email a completed form and include attachments? The designated recipient of the email needs to be able to read a formatted email, not have to import form contents into another app before being able to read it.

not really sure what you mean, but phpmailer on sourceforge is more satisfying than a big pot of tea, 10 ciggies and a bitch about Jeanette on reception

An employee of the site will fill out a web form (expense). Once the form is completed, the employee needs to be able click on a email button to send the form contents and potentially attach additional files to a defined recipient.

So my question is, how can the form be sent so it retains its formatting (e.g. converted to a .jpg before sending???) and is it possible to add attachments before the email is sent. Please note that this form could be filled out anywhere in the U.S./Canada where there is a computer with internet access.
