Getting uploaded filenames and putting them into a database

Hi all, Sorry if I'm asking a stupid question I'm a complete newbie. So please bare with me.
Basicly the situation is that I'm using David Tufts fileupload class to move files using http. I got that working but what I want to do now is to take the name of the file that will be uploaded and them pass into a mySQL database.
I tried nesting forms but that doesn't seem to want to work.
Any ideas?

the filename will be in the $FILES array under the field name you used in the form. ie: $FILES['field_name']['name']
