file upload to database

[PHP-ing]

I've done a simple uploading page which allows the file to be uploaded to the directory. But, what i need now; is to add the file path to the database (Mysql) table. After some research, i realised its better to store the path of hte file in the database instead of hte whole chucnk of information.
is there anyway some kind soul could help me out with it?

Let me ask you what exactly don't you know? inserting a new values(new rows) into the database? or something about uploading?
Please clarify.
