customize reports mysql php

hi masters. i need help in customizing fields to be display. For example i have an
ESTABLISMENT table having
10 fields
id, estab_name, address, city, zip, date_estab, manager_name, insurance_company, org_type, region .

How am i supposed to display these fields in a table format in a user defined data.

For example the user could only wants to display the estab_name, manager_name, org_type and region.

I would use a checkbox array. Set the values of the checkboxes to the names of your columns. Then just use a for loop to generate your query.
