Upload Script

I've looked online for a long time now and I can't seem to find a very simple upload script. I have the form but I can't get the actual file to upload. Does anyone have a real simple script that will just upload one file to a web server? Thanks for your help in advance.

Where can I get a file that all I have to do is change the location where the file is uploaed to?

The file upload location is specified in your php.ini
