Help with uploading files

If anyones good with uploading files, Im trying to create a form where multiple files and text from a text box can be uploaded at the same time possibly indexed into a database for each pic to correspond with the text submitted? has anyone done this before, I used the code from http://us3.php.net/features.file-upload (muoihv at 1yt dot net) which worked but im still clueless as to how the text box will work with it, thanks

PHP file upload is a multipart/form-data upload. That simply means you can mix text fields with files. You can access the files with the $FILES array. And you can access the other form data with the $REQUEST array.
