Help with uploading

Hello everyone,
thanks for your help for my earlier post.

I will like assistance regarding this issue:

I've 2 forms to maintain, one is an "add client" form and another is a "view client" form.

If a client(remote user) chooses to upload an image file onto the server(as an example) in the "add client" form, how can I code in such a way that the image actually appears in the "view client" form?

It's quite tricky and I'm really new to PHP, your advice is really appreciated!

Base Logic:
1.
During Add Client
Upload File into temporary location, rename file with Member ID saving extension
During View Client
See if File exists with Member ID, display img tag if exists

2.
During Add Client
Create a new folder with member ID and upload image into this folder
During View Client
Check if directory exists, display img tag and src = "images/memberid/theonlyfileinfolder"

Spend sometime on filehandling function on php.net, its worth it.
