allowing visitors to upload files

Hello..
I try making form html for who can send to me movie or image on our web then I check my web FTP who was upload file. must be use php on base data. I dont know how use base data with php or sqi.

<form method="post" action="www.qsuno.com/ ? or php ?"> mean put Pubile_html in the FTP ? or put php...

<form method="post" action="upload.php"> that mean upload.php where put ? And I dont know what html code in the upload.php ?
I know only upload.html mean is

<form name="upload" method="post" action="upload.php" enctype="?">
<input type="file" name="Image"><br>
<input type="submit" value="Upload Image">
</form>

And enctype="?" what I do code html ?

I need to know three things 1.upload.php
2.upload.html
3.enctype="?"
