Script Recommendation - Dynamic Form Generator from MySQL

Hi,
Can somebody recommend a script that dynamically builds a form from data (or really meta data).
So I have a MySQL table called 'HOUSE' with 5 columns. I would also have 5 rows in some other table with data that would be used to build a form for the data retreived or sent to 'HOUSE'.

I'm certain somebody has some of these.. but I can't seem to find any?

Please don't ask me to search Google or HotScripts. I did with no valuable results.

have you looked at QuickForm under the PEAR library. pear.php.net

might be worth a shot
