Code for Uploading and Viewing a Profile with picture on a MySQL database

I am trying to create a database where people can add information to a profile like their name and birthday and bio as well as a picture. I am not able to get the upload to work or am able to view the uploaded profile can some one give me some help or some code on how to upload profiles with informaiton and a picture and help or code on how to view the profile after I upload it
