Mail sent by: undefined <undefined>

Morning All,

I am a complete novice at PHP but am trying to learn.

I have recently worked through a tutorial to create a flash form driven by PHP.

Thought it was all working but when I click send and check my email inbox, the subject is 'undefined' and so is the from.

Can anyone help me please.
