Form not reading checkboxes

I had a wonderful partner who did all the programming for our site. But she has a full-time government job now (good for her!)

I transfered the code she had written into a new page layout and now my form doesn't read the checkboxes. The code was designed to read both checkboxes and textboxes and print out the results.

Here is a sample of the old page that still knows how to read both checkboxes and textboxes

Here is the new page that only remembers how to read the textboxes

Can you see where I have left something out, or put something in the wrong place on the new page?

My problem was solved by a helper named Butcher on another forum. I had placed a google search box on my page and the two forms could not work together. I took out the search box and now my visitors can once again create their own shopping list!
