itrep, you've got to meet us half way here - read what we write. Your question has been answered at least three times already in this thread. If you don't want to do it yourself, fine, I understand that, then hire someone to do it for you. But if you want to do it yourself and learn a little about PHP, you've got to at least try the suggestions we make.
You know the home page works so you're 99.99% of the way there. All you need to do is copy the part that works and replace the part that doesn't work.
At the top of your home page (which works) you have:
<? include("header.php"); ?>
And at the top of your basketball page (and all the other pages), you have:
<? include("http://www.itrep55.com/header.php"); ?>
Do you see a difference in those lines of code? Please forgive my frustration, maybe I've had a long day, but if you're going to ask us for help, you need to at least try our suggestions.
Oh, here's one thing: include commands include files that are in the same directory. If the basketball index page is one directory lower than you home page (which it is), then you will need to tell the include page to get the header from the directory above it by putting in a ../ like this:
