Hi, I was wondering if someone would be kind enough to help me develop a php page like this one: http://belkihost.com/step_one.php? so that they can choose how to pay and choose a package, which their order details will be emailed to me. Help would be greatly appreciated.

I think that you should TRY to make one first, then we can help you with any errors that you come across. Someone may make one for you, but most people (including me) do not have the time, so please create one and we will be happy to help you make it work and help you with how to make things happen.
