Need help looking for specific script or application

Hi there,
I have some pretty specific requirements and am looking for a pre-built script or application as a starting point instead of starting from scratch.

My requirements are to ask a series of questions. The answer to the current question determines what the next question will be and so on until the user arrives at an endpoint having answered all questions correctly.

I'm not sure how to search for this. My searches so far lead me to survey and quiz and both don't really meet my requirements as they cannot control the flow.

Any ideas and pointers in the right direction will be most appreciated.
