Scoring system for checkboxes

So I have a form which acts as a self assessment tool. I've been asked to develop so points are allocated to each checkbox. The score needs to be kept too and outputted at the end of the assessment. Ive looked on the archives and seen some remblance of resources but i have no idea where to start.

here is a snapshot of my form:

                    </tr>

in this case I would like it that 5 points be awarded for responding to the first option, 3 points for the second option and 0 points for the third etc.

have you used PHP to do form handling before?

Have you even used PHP before???

If you save that as process.php, and set the action of the form above to "process.php" put the two in the same directory, and it should work!
