[RESOLVED] form appears to not work?

Got some of it figured out, the page loads, but now the form doesn't seem to be working at all now, can someone take a look at the link?

U forgot to delete a bracket ...

I don't see any problems there? That's why I said that is working after you posted, to delete a bracket.
