FTP Sing up via php

i need a program or php script that will allow users to sign up for an ftp account via a php page (figured php would work best caus all singup pages now are .php)

it would look like

[there userame]
[there password]
[size selection ,10mb ,20bm ,50mb ,100mb]
[there email]

(signup button)

and i would make a ftp account on filezilla or any ftp program that would support it.

i have no idea even where to start 😕 😕

you should use a form submit page
and let PHP take care of data submitted

what you should decide is what your script should do with submitted data:
- save, write all sign ups into a file at your webspace
- save into a Database, like MySQL
- send you an email for each sign up
