OK, When I press the project button on the index.html I get this message.

I know that this says that the password and login are incorrect, but I have checked and, I have copied and pasted the user names and passwords in to the forms.
The login.php page should open and the person should login and then go to the project page and they click n their account their access level. All of the tests have connected to the server, and all of the passwords and user names are in place. I thought that this was a host side error because the username and password are correct and "cluster and affinity" are in the message. Since there is nothing between the index and php pages, I just don't where to look for an error. Dreamweaver 8 demonstrates the connections to be sound.
I have attached the codes in a doc file. . . I sure hope someone can help.
