Display Category then subcategory

I have never had to this before or ever really learnt how to do it, so would be interested in finding out how you go about displaying categories and then the subcategories from mysql in php.

I have seen this done on some holiday rental websites where they display the country and then all the regions underneath.

Example of store:-

PHP sportswear - (Main Category and subcategories)
- Football boots
- Football shirts
- Football shorts
- Football socks
- Footballs

JSP sportswear (Main Category and subcategories)
- Football boots
- Football shirts
- Footballs
- Shin pads

I know how to indivodually display results whether it is categories or sub categories but how do I combine both to be echo to screen as above.
