How to create dynamic check boxes from MySQL DB?

Hi,
I have 4 table in MySQL Database:

• test_2

• test_3

• test_4

all tables contain 2 field from 4 to 8 record.
ok?
I want to create dynamic "check boxes" and put MySQL table information into (X)HTML Table with split informaton to 3 column.
