Select date from calendar

I'm creating a simple form to collect some information from visitors, however i'd like to use a small calendar where the visitors can select a specific day. Can anybody provide me some links for tutorials for this thing?

Here is a nice DHTML/Javascript one I tend to use a lot or if you want a php one then try Hotscripts, they have around 13,000 php scripts available.
