multi-line form input to mysql table - help, please

(first post, with fear and trepidation...)

This thread almost answers my question -- but I'm sufficiently new that I can't see how to modify it to fit my needs.

How do I write a form with multiple rows and columns that will read into mysql, line by line?

The form needs to look something like this:

site | latitude | longitude | 5cm Temp | 10cm Temp | Infiltration | etc...
-----|----------|-------------|--------------|----------------|--------------| -----...
 #1 |            |                |                  |                    |                  |   ...
-----|----------|-------------|--------------|----------------|--------------|-----...
 #2 |            |                |                  |                     |                 |       ...
-----|----------|-------------|--------------|----------------|--------------|-----...
 #3 |            | ...
-----|----...

...for 10 rows and 14 columns of input that will read, row-by-row into a table.
(those are supposed to be empty input boxes between the pipes following #1, #2, etc -- am I missing a way to force fixed widths in a post?)

(Am I asking in the wrong forum? Describing the problem poorly? Missing something so obvious that you all can't bear to embarass me by pointing it out?)

u may use arrays of form controls which can be populater using loops after submission..
