upload fourm

ok i am tring to make a php upload script so users can uplaod thing to my sever

here are all the code i have made

but is dose not work can any help me get it work

here is the HTML code to start it all off

and here is the php i have done

$target_path = $target_path . basename( $_FILES['uploadedfile']['name']); 
$_FILES['uploadedfile']['tmp_name'];

this the temp php code to check the file i think i have found this on the
net and i am tring to get it to work for my site

here is the last php code then one saves the file to the sever

$target_path = "uploads/";

$target_path = $target_path . basename( $_FILES['uploadedfile']['name']); 

if(move_uploaded_file($_FILES['uploadedfile']['tmp_name'], $target_path)) {
    echo "The file ".  basename( $_FILES['uploadedfile']['name']).

} else{
