PHP tabke layout

Dear all,

I have just started learning PHP/MySQL. I am at the stage of creating a database but still have some problems with PHP layouts.

I would like to create an address book similar to the picture below, name/add on the left side and phone numbers on the right side.

Can you guys please help me and give me some hints??

right the the most difficult part is to output the data to the table shown in the picture...
I can display everything in 1 single row.. but how do I display the address below the name? and the 2nd telephone number below the first one??
I tried google and couldnt find much of helps..

Please have a look and let me know, thanks

Ps, for the address, i think i will have to add a few more tables like "street" "Postal code/region" "Country" in order to display them in different rows
