Submit form to email

Hi guys,

I’m kind of stuck with the following item. Any help would be great……

Basically I hoping to build a form, with the following details:-

Date of Incident
Time of Incident
Name (optional)
Address (optional)
Contact Tel. No. (optional)
Contact Email (optional)

Question1
Answer1

I want this information to be sent to an email address. I know it would be better to sent to database, but I want it to be sent to email address.

Any help would be great!!!!

What is your PHP level? Sounds like a beginners problem, unless you can show me some code which you are stuck with.

In principle:
= > Form with fields, which submits to itself, or a new page. New page is easiest if you are just starting.

=> In this page you get the variables from the POST array, and create a variable which holds the email body.

=> Use the mail function to send the email.
