Php/ Mysql export information

I have a website now I am making with a backend database. I have a few forms on it that have users fill out information and select from drop downs and check boxes. I want to be able to capture the data the enter and store in a DB that I can login to and see from the admin area or export to an excel file and view. This is a lead generator website.

I would then look at the data and be able to add in if it is a good lead or dead ect. What is the easiest way to do this or to start it?

do you know how to create a database? if not, you should probably start there.

Ok, but I do know how to build a db, this is already done I just needed help with building in the export to a excel file and be able to specify the fields I want also.
