Form Validation

I'm looking for a simple form that looks like this:

First Name, Last Name, Email, Question.

Simple feedback form. I need this to validate the required fields(all) and then submit the form to salesforce.com., and also email me to tell tell me a submission occured.

Does that make sense? This is turning out to be very difficult to do in a simple, clean way.

By validate do you mean, filled in, or entered in a certain way like the email field requires [e-mail].

Well, validate...First Name and Last Name with text and email with proper email format.
