store data from the form field

Dear all,

i am newbies on php. want to ask some question. thank first for the help

i have a php website which now add the form which has 5 fields (Name, telephone, email add, postal add, and comments) to let customer to enter their information when they are visiting my website. any simple php code can share to me to store those 5 field's data everytime to MS Access file or mysql db? (MS Access file i preferred for later easy to read it. if not, mysql is also welcome)
