This is more a php problem than a db problem. The "index" being referred to here is an array index. A couple of hints though.
1: always quote literals for array indexes. i.e. this: $arr1['city''] is correct, but this: $arr[city] is not. Unless you've defined a constant earlier that start stands for, which is not that common, and if you did, you'd know it. 🙂
Look at what's on line 13 of that file. In fact, post that line and few around it here for us to look at, and enclose it in php tags. I'm guessing you're just trying to reference a non-existant array index. But I'm not entirely certain.
Note that these aren't errors, so you're probably not in that much danger.
