form to database.. to email reminder?

What I'm trying to accomplish:

1. Have a simple form that will be filled out one time each person receives a new Group Id (this will be a basic code that is the same on a set of cards). After the form is complete it will be sent to a database.

2. The database will have to be accessed (hopefully this will be automated) to retrieve the users with the same Group Id.

3. We or Software will send out emails (one time) to those groups a few months before their member cancellation.

4. This is not for logging in. It is a form to database to email reminder. It would be nice to find software that will automate this; so far I can only find php forms that do the first part of the job - form to database.

5. The only thing that may be tricky is I need it to email only people with the same Group Id before their member cancellation instead of everyone in the database.

Please let me know if anyone has ever dealt with this or can point me in the right direction.

Sounds like you're looking for a canned solution and maybe you have some options there. But this would also be a fairly straightforward custom dev project. Do you have enough experience to give it a shot?

unfortunatly I have some experiance with, but no knowledge of PHP I would like to find something in which I can easily make changes. I am weak on my programing, but have used a login system before which required me to create a database for MySQL. I have created several sites also. I just need something that hopefully I wont have to change alot of code. I can make out and understand a few things to change values for tables or user data. I hope my inexperiance is not confusing anyone. :bemused:
