PHP to MS Word works, but how do I format certain elements?

I have generated a Word doc from PHP and made the form look just like the original. Now, how do I do these things:

Insert images
Change page margins
Insert Header and Footer with page numbers
Insert page breaks (already looked at CSS Print Ref from w3schools)

If anyone can think of other tips and tricks that I have not mentioned, that would be great as well. And I do NOT want to edit this in Word. I want this generated from the web page. I had to mention that, because I have seen too many replies to other requests like this where people tell them to edit it in Word. That totally defeats the purpose of generating it from PHP.

What method are you using to generate the word documents? COM object? a PHP library?

I'm generating it with the header tag, since the site is on a *NIX box. Alas, no COM.

Outside of COM I have never seen a library for generating Word documents directly. There is one for excel but as soon as you want to do something a bit fancy then it no longer works well.

But as long as you are using Office 2003 or later then make a page using Word and save it as xml. You can then use the xml as a template in your php.

Finally moved to a Windows server. Now where is that pesky information for COM headers, footers, page numbers, etc.?
