Requirement:

2. At least 2 images using the echo statement.
3. A form using the heredoc syntax that will ask for:

5. mi - middle initial
6. lname - last name
7. gender - using option buttons
8. emai - email address
9. message - text area
   The target of the form is the same file. Simply echo the values entered in the form.
