The pear package looks interesting enough. Basically you need a database of IP->country relations, that is current, and keep it up to date.

Define "real" ? If you're wanting the internal address of a computer behind a router, you're not going to get it for a couple of reasons:

It's internal for a reason - the outside world doesn't need to know it. I don't want my networking equipment to start leaking information about my internal network setup!
Even if you got it, it wouldn't do you any good. At all.

Yes. $_SERVER, not getenv.

How you use IP Address to determine the country?

Get an ip-to-country database (these are available for free) and do the required lookups. There are examples around.

It's not 100% reliable though.

cos _SERVER won;t work, it say unknown IP address or 127.0.0.1

The client IP address is always known. $_SERVER['REMOTE_ADDR'] can NEVER return "unknown ip address" on a working PHP installation. It will not return "127.0.0.1" unless the client or next hop is the local host.

The client IP address may not be the final one, but it will be an IP address of a hop somewhere in the process and is sometimes (but not always) a good indicator of the end user's country.
