hmm, have you checked different browsers to see if the same thing is happening, apache/php will put out the same html so it comes down to browsers. there are alot of differences between PHP4 and 5 and apache 1 and 2, so I dont know if any of those are responcible, but check browsers as they tend to give you insights into how the html is being handled
