Hi,

I have been looking for a long time for a working upload form

My webpag visitors should:

Complete a form (Name, website, email address) + upload an image file.

I really haven't found a working script on the net. Maybe someone could help me.

I would like the script to look like this: CLICK ME

Firstly look at this thread

You need to get the basics at least when attempting anything with php, you must start at the beginning as we all have done then build you working knowledge. When you encounter problems then you can post your code so that members can assist. Take a look at the manual at http://www.php.net/ sorry to be blunt but if I had not said this others will have done.
