Simple Rating System

Is it possible that someone can give me the php coding for a simple rating system.

I don't like "star" rating systems but a simple radio button rating system from 1-10, but a star rating system will work.

It justs needs to simple, something that goes by IP address so users can't vote twice, and it needs to be so I can add it in a small section of any site page I need to.
