Apache bench question

Does anyone know the meaning of "Failed request" in ab result? What could cause this failure?

You may have your mod_evasive or mod_security settings too tight. AB sends requests at a very high rate of speed, and those two mods are designed to stop such things.

Are you using either of those two mods? Perhaps your hosting provider has a cap as well...

I use the default setting for ab, since I don't really know way to use it.
