Age old date/time conversion Question

I know this is something addressed left and right, however, as a newbie, I just can't seem to get it straight.

I have my mysql db that has the date stored in the YYYY-MM-DD format. I also have time stored in HH:MM:SS (24 hr) format.

Using PHP on my website, I would like to be able to show the date in mm/dd/yyyy format and time in 12 hour format. How can I do this?
