PHP Form - Error message problem

I am completely new to PHP and have been using a program called Forms to Go to generate my php files.

I don't know if its to do with the html in my form or the php coding but for some reason although all unfilled fields in my form should have an error message, all the list menu fields are not being included in the error page list when the incomplete form is submitted.

Can anyone help?
