script installation help

Hi i have a script that im trying to install but cant get working

MySql Server
Database Name
Mysql Username
Mysql Password
This Site`s Name
Your E-mail
The URL

What do i put in for the mysql server??

Usually it is localhost

If you have access to phpMyAdmin, login to it and it will tell you your server location.

If localhost doesn't work, try using your server's IP. A quick way to find that using Windows is to click Start >> Run... and type CMD, then press enter.

When the command prompt comes up, type ping mysite.com and press enter. Your server's IP address should come up in the ping results.
