Uploading path file name to SQL database

I am using the following code to upload images to a folder within my directory. How do I manipulate this code to add the uploaded files path name to MySQL database?

Upload form:

I think something like

$query = "UPDATE Your_table SET photo_id = '$target_path' WHERE id = 'your_specific_record_by_id'";
