[RESOLVED] Failing to add information to database...

Hello everyone, I am trying to add information to a database... I have build a form and another file that I will show below, but it doesn't add any information to the database, and I am not sure why... can someone tell me what I am doing wrong?

File 1: adicionar.php

You are not connecting to your database, so I am sure you would be having a error spat at you.

Change this:

mysql_connect ("localhost", "user", "pass") or die ('Erro: ' . mysql_error() );
mysql_select_db ("tracking");

to this:

		$user = "user";          // Database username
		$pass = "pass";                // Database password
		$db = "tracking";       // Database name

mysql_select_db("$db");
$ok = mysql_select_db("$db");

}

I would even remove this from your script altogether and add it with an include.

So save the above code as dbcon.php or whatever you want.

Then include it in your script at the head

?>

I am sure you will be up and running with this minor change.

Thank you so much for taking the time to answer and try to help, however, it keeps giving me a blank page after I click submit on the form and when I check with the database all fields are still blank... so I still need help, but thank you so much for taking the time!

I still can't see where the mistake is...😕

Replace
