Generate Word Templates from PHP

I am trying to 'Generate Word Templates from PHP' can this be done?

I have a basic Word template and I would like, if possible, for the users to add their name, address, telephone number and email into a form which would then some how appear in the template, ideally in the footer.

Can this be done if so how please, any help would be appreciated?
