PHP Email Form

I need help creating a form that will send and email out in php. It needs to have a drop down with several options of emails a user can select to email too. I have a screen shot of the fields i need. I hope this is easy and that someone can point me in the right direction as i have no clue what im doing.

Thank you so much for that code. But I need this to be a little more detailed as i need there to be a drop down to where address will appear to send the email to. YOu select the address you went the message to goto in the dropdown meanu and type you message and the message will be sent to.. Hopefully someone has some ideas on how to go about this.

Sure: add the desired drop-down to the form, and in the processing section retrieve its value, validate that it is an allowed value, then use it for the "To:" email address. (This is, after all, supposed to be a forum to help you learn to do PHP, not a free script repository. 😉 )
