how do I create a database with the users name?

I am looking to make a webpage that will create an address book. I think that I need to make the table inside the database named Numbers, somehow to pull up only their information. So say I logged in, and added some people I knew into the database... then I searched for it. I don't want to see every single Tom that everyone on my site has put in. I only want to see the Toms that I added. So my question is, how do I make it so that it will only show me MY own added information?

to the table add a "added_by" field then when searching add a where clause to only search where added_by = the searcher.
