Server Related Error in Script

Hey all, n00b to PHP here. A coder wrote me a script about a month ago, and it worked just fine on his server and on my personal server, but when I uploaded it to my company's webserver it doesn't function properly.

Basically how it's supposed to work is you fill out the fields at http://1minloans.com/roman2/admin/form.php and then you can view a set of tables that have been filled in with the info at http://1minloans.com/roman2/ by selecting one from the dropdown menu.

Whenever I fill out the fields and select and image to upload, I get an error like this:

Error: please fill in all fields
insert image name here

I've got GD enabled, so I don't know what is going on. My phpinfo is located at http://www.1minloans.com/phpinfo.php

Can somebody please help me?

At a guess, the servers where it worked had [man]register_globals[/man] turned on, perhaps? If so, a quick fix would be to stick in a call to [man]import_request_variables[/man] at the start of the script. (This is a kludge to work-around this dependency on register_globals. The real fix is to reference the form variables via the $POST or $GET arrays as applicable.)
