What does this mean?

Warning: Cannot modify header information - headers already sent by (output started at C:\xampp\htdocs\authenticate.php:3) in C:\xampp\htdocs\authenticate.php on line 39

I have copied the files that came on a CD with my book and ran the example and this error came up? It is a log in script and the database is set up properly. I'm running the latest version of XAMPP from Apache Friends.

Well, this is not a database error, it is a PHP error. Hopefully some moderator will move it to a more apropriate forum.

What it means is that you try to set headers after you have sent something to the browser. A space or return is enough, you can't send anything before the headers.

A header is instructions to the browser that won't be displayed on the page. They are sent from within the PHP code.
