drop down menu form

Hi,
Two questions:
1-How do you make a form that has a drop down list of items to choose for a field? Just some basic guidance needed.
2-Based on the selections of that drop down menu form, I would like to display something (a calendar) however before the user selects a choice, I would like one choice to be displayed by default. How can you make that? (i.e. calendar for Nov. 2008 for events in new york only)
3-Can I also make random display in the beginning instead of a fix default display like in item2? If so how? (i.e calendar for any month for any city and state) Or better yet, display the calendar for the users geographical location (in that case how do you determine the general location of a user from his ip)

3th bullet:

the easiest way, if you create a login form, and select his/her timezone.

or you have to create an IP-> country location converter script.
