PHP Notice: Undefined variable:

I'm getting a problem with my PHP I'm guessing.

Any ideas on what could be wrong? my register_globals are turned off, I've turned them on and I get the same thing.

In the first case it's telling you that you are trying to append (.= operator) to the $html variable, but that variable does not yet exist. At that point PHP creates it automatically, but the notice is generated to alert you that you may have misspelled the variable name, or should be assigning ( = operator) instead of appending.

In the second case it is similarly telling you that you are trying to use the undefined variable $failure. Since it's undefined it is treating it as a null value within that string. It could be something simple like a misspelling (remember that variable names are case-sensitive), or there could be some sort of logic problem or other error that is causing the portion of the script where it's supposed to be defined to not be executed.

I've uploaded it on my server at home, nothing seems to be wrong with the script, I'm thinking that it has to be something to do with the php setup...
my php at the house uses php-5.2.4-3

it just loads blank pages on my remote server.

The difference is probably the error_reporting level. Since it's a notice-level warning, it won't show up if the error_reporting level is not high enough (or if display_errors is turned off). To make sure, add the following lines to the start of the script:
